Precision Med & Wellness LLC (“PMW”) — Terms and Conditions

By using the PMW website or scheduling or receiving services, you agree to these Terms and Conditions, subject to rights that cannot be waived under applicable law.
Services
PMW provides mobile specimen collection and wellness services as described at the time of scheduling. Mobile blood draws and specimen collection are available across the Greater Tampa Bay area, subject to location, appointment availability, service eligibility, and laboratory requirements. Please confirm your specific collection and location before booking.
Payment Terms and Conditions
Payment is due at the time of service. Review the service price and any disclosed additional charges before confirming your appointment. Mobile collection fees and laboratory testing fees may be separate. PMW does not guarantee insurance coverage or reimbursement; confirm testing charges and coverage with the laboratory, provider, or insurer. Separate provider agreements may establish different payment arrangements.
PMW reserves the right to update prices for future bookings. Changes will be disclosed before you agree to the affected service; website updates do not automatically change an already agreed appointment price.
Appointment Terms and Conditions
Please provide at least 24 hours’ notice when cancelling or rescheduling an appointment. Cancellations made less than 24 hours before the appointment are subject to a 25% cancellation fee. Contact PMW if you have questions about cancelling or rescheduling.
If PMW needs to postpone or cancel a visit, PMW will contact you to discuss the next step. Availability of online appointment times does not guarantee that every collection, location, or laboratory requirement can be accommodated.
Client Responsibilities and Consent
Clients are responsible for providing accurate information and a safe, appropriate location for services. Provide required lab orders, collection instructions, or other documentation through the channel PMW designates. Follow the preparation instructions given by your ordering provider or laboratory. Do not change medication or begin fasting based solely on this website.
Scheduling or using this website does not replace procedure-specific consent. An authorized representative should contact PMW to confirm requirements when arranging services for another person.
PMW’s Right to Decline or Stop a Service
PMW reserves the right to decline, postpone, or discontinue a service when required consent or documentation is missing, preparation requirements are not met, the requested service is outside PMW’s scope, laboratory requirements cannot be met, or the location or circumstances make proceeding unsafe. This includes threatening or abusive conduct that affects safe service delivery. PMW will explain the issue and discuss an appropriate next step when practicable.
Specimen Collection and Testing
The applicable laboratory determines whether a specimen is suitable for testing and is responsible for processing, analyzing, and reporting results. PMW is not responsible for testing that is not performed or cannot be completed for reasons outside PMW’s control.
If a specimen cannot be tested due to an issue attributable to PMW’s collection or handling, PMW will work with the client to determine the appropriate next step, which may include recollection when necessary.
PMW does not interpret laboratory results or provide medical diagnosis or treatment. The ordering provider advises you about results and your care. Laboratory turnaround times and result delivery are outside PMW’s control; PMW does not guarantee a particular result or delivery date.
Privacy and Communications
Review PMW’s Privacy Policy. Do not send laboratory orders, diagnoses, insurance documents, or sensitive medical information through ordinary website inquiries, email, or text. Call for instructions on providing required documents.
If you opt in to texts, messages may concern appointment coordination and service updates. Message frequency varies; message and data rates may apply. Reply STOP to opt out and contact PMW for help or another way to coordinate services. Marketing messages, if offered, require a separate opt-in and are not a condition of purchasing services. Carriers are not liable for delayed or undelivered messages.
Website Information and Emergencies
Website information is general and does not replace advice from your healthcare provider. PMW’s website and scheduling service are not emergency services. For a medical emergency, call 911 or seek immediate emergency medical care. Linked scheduling and other third-party services have their own terms and privacy policies.
Changes to These Terms and Conditions
PMW reserves the right to update its services, pricing, policies, and these Terms and Conditions as needed. Updated information will be posted on this website with an effective date. Changes apply prospectively, subject to applicable notice requirements and existing service agreements.
By scheduling or receiving services, you acknowledge that you have reviewed and agree to these Terms and Conditions. Nothing in these terms limits rights or remedies that cannot lawfully be limited.
