PMW respects your privacy and is committed to protecting your personal information. This policy explains PMW’s handling of information provided through inquiries, appointment scheduling, and services. However, it does not replace any separate Notice of Privacy Practices that applies to protected health information.
Information PMW Collects
PMW may collect information you provide when you contact PMW, schedule an appointment, complete forms, or receive services. For example, this may include your name, contact information, service location, appointment information, payment information, and health information needed to provide services.
PMW’s hosting and scheduling providers may also process technical information, such as IP addresses, browser or device information, cookies, and service interactions, to operate their systems. In addition, cookies and similar technologies may support functionality and, where enabled, usage measurement. Also, you can manage cookies through your browser settings; disabling them may affect certain features.
How PMW Uses Your Information
PMW uses your information to schedule and provide services, communicate with you, process payments, coordinate laboratory services, maintain business records, and comply with applicable laws.
HIPAA and Health Information
When applicable, PMW protects and handles protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and other applicable privacy requirements. In addition, PMW only uses or discloses health information as permitted or required by law or as authorized by you.
Using PMW’s website or scheduler is not authorization to use or disclose medical records for marketing. Do not send laboratory orders, diagnoses, insurance documents, or other sensitive health information through ordinary inquiry forms, email, or text. Call PMW for instructions on providing required information through an appropriate channel.
Information Protection and Retention
PMW uses reasonable safeguards to protect your personal and health information. However, no internet transmission or storage method is completely secure. Also, PMW retains information as needed for the purposes for which it was collected and applicable business and legal recordkeeping requirements. In addition, PMW does not sell your personal information.
Third-Party Services and Laboratory Coordination
PMW may use third-party services for scheduling, payment processing, communications, website hosting, and laboratory services. Information may be shared with these services only as necessary to provide services, as permitted by applicable law, or as required by law. PMW may also share information at your request or with your authorization where required.
PMW uses Realeflow’s website platform and links to Acuity Scheduling for appointments. However, those platforms and other linked services have their own terms and privacy notices. Therefore, review the applicable notices before providing information.
PMW does not share mobile numbers or text-message opt-in information with third parties for their marketing or promotional purposes. Messaging vendors may receive information needed to deliver messages on PMW’s behalf.
Communication Preferences
Promotional communications are optional. Marketing consent is not a condition of purchasing PMW services. Reply STOP to opt out of texts, use an unsubscribe link in promotional email, or contact PMW to request that marketing communications stop. Call 813-699-9344 for help or another way to coordinate appointments. In addition, necessary service communications may continue through an appropriate alternative channel.
Your Information Requests
Contact PMW to request access to, correction of, or deletion of your personal information. PMW may need to verify your identity and may retain records required by law or necessary for applicable legal obligations. Applicable health-record rights are addressed under the relevant service arrangements and privacy requirements.
Children and Authorized Representatives
PMW’s website is intended for adults requesting information or coordinating services. A parent, guardian, or other authorized representative should call PMW to confirm service eligibility, consent, and documentation requirements for a minor. Do not submit a child’s medical information through ordinary inquiries.
Changes to This Policy
PMW reserves the right to update this Privacy Policy as services or practices change. PMW will post the revised policy with an updated effective date and provide additional notice where required by law. However, a policy update does not expand uses or disclosures of health information beyond what applicable law permits.
